(Instructions: To be used by a public accommodation after they have modified policies and procedures and removed architectural and communication barriers. A form similar to this may be used and a procedure set in place to provide an avenue for anyone with a disability to make a particular accessibility problem known. This allows the person with a disability to see that those responsible for the facility are making a good faith effort to provide access in their facility. The back of the sheet may be used to track internal routing, approvals and follow up calls according to company policies and procedures.
