I use Dragon NaturallySpeaking (formerly Dragon Dictate) and have for many years. It is amazingly fast and accurate and it requires only a little training to get it started with your voice. The more you use it the better it works. It can also be used to control software, too. I often use it by dictating into a digital recorder while I drive and then having it transcribe the recording when I'm back in the office. There are multiple versions for different types of devices with basic to advanced capabilities depending on your budget and needs.
